Residents are admitted to Daleview Care Center 24 hours-a-day, 7 days-a-week. We are always available to assist residents and families with the transition through the admission and discharge process. Our active social services department works closely with both resident and family councils, meets with families to discuss discharge planning, and coordinates home care services.

Once admitted to Daleview Care Center our professional staff will customize an interdisciplinary care plan for each resident to assist them in living as fulfilling a life as possible in a caring and nurturing setting.

Our total focus is on the health and well-being of each resident in our care.

Daleview Care Center accepts residents requiring long term care, short term rehabilitation, subacute care and other types of health services.
