Mike Lugo, III, LNHA
Administrator
I am a Licensed Nursing Home Administrator with deep roots in long-term care. Born into a healthcare family, I have spent over 20 years immersed in the field and has worked professionally in long-term care since 2018. This lifelong exposure has shaped my commitment to compassionate, resident-centered leadership and operational excellence.
As Administrator of Daleview Care Center, I oversee daily operations, regulatory compliance, and team development, fostering a culture focused on quality outcomes, dignity, and collaboration with residents and families.
Prior to becoming an LNHA, I served in the following roles within long-term care facilities:
Human Resources Clerical — 2018-2020
Director of Employee Health—2020-2022
Director of Medical Model Adult Day Care — 2021-2022
Assistant Administrator — 2022-2024
I hold a Master of Business Administration from Molloy University and am currently pursuing a Doctorate in Public Health, focusing on strengthening systems that improve outcomes for aging populations.
I am passionate about empowering care teams, supporting residents and families, and advancing best practices across the long-term care continuum. It is my absolute pleasure to serve the residents of Daleview Care Center and our local community.

Wendy Rodriguez RN BSN WOCN IP DNS
Director of Nursing
My name is Wendy Amoy Rodriguez, and I am proud to serve as the Director of Nursing at Daleview Care Center. This role reflects not only my career growth but also my lifelong commitment to caring for others.
I grew up in a family where caring for others was a way of life. My mother’s compassion and dedication inspired me from a young age and shaped the values I carry with me today—empathy, responsibility, and service. During my school years, I developed a strong work ethic and a natural desire to help others, which began to shape the direction of my future.
Before nursing, I worked in retail management, where I learned leadership, organization, and problem-solving. Choosing to enter healthcare was a turning point that allowed me to align my career with my passion for helping people. I committed myself to nursing and continued to grow both professionally and personally, building a strong foundation for the path ahead. I joined Daleview Care Center in February 2022 as an RN Supervisor and later became Assistant Director of Nursing. During this time, I earned certifications in wound care, infection prevention, and nursing leadership. Today, as Director of Nursing, I take pride in the relationships I’ve built and the care we provide every day.
The values I learned early—empathy, integrity, and compassion—guide everything I do. I strive to create an environment where residents, families, and staff feel respected and supported. I’ve learned that leadership is about connection, trust, and growth. Challenges have taught me to stay adaptable and focused on what truly matters—people.
Today, my focus is on supporting my team and ensuring our residents receive the dignity and care they deserve. I aim to continue strengthening our team, expanding staff development, and improving the quality of care we provide. I am grateful for the journey that brought me here. Daleview is more than a workplace—it’s a family. I look forward to continuing to lead with compassion, purpose, and dedication.

Renee_LaPlaca
Administrator Assistant
I have been at Daleview for 4 years as the Administrator Assistant. In addition to my most important role I oversee Reception, Medical Records, Human Resource, and Central Nursing Supplies. Born in Brooklyn but moved out to Long Island when I got married in 1988. During the course of these 22 years, I worked in management at Gurwin Jewish Geriatric Center, The Jewish Home Life Care in the Bronx in operations, Hertlin House in Ronkonkoma as a Food Service Director. and Director at Harbor House in Port Washington. But my last stop and forever home is Daleview Care Center as an Administrators assistant.
Today I am a Nana, a mom, I joined a ministry, I love a lot of things but more than anything else I love to see smiles. I am blessed with good people all around me. Daleview has an incredible team of great people, great residents, great dynamics I wouldn’t have it any other way.
If I were to give a closing reflection as a preschool teacher and Administrator Assistant I have seen the joy of the beginning of life and the importance of joy at the end of life. I’m thankful to bring Joy where I can for my residents.

Deslandes Cange, RN BSN IP
Asst. Director of Nursing
I graduated from the university of Grand Canyon with my BA in Science & Nursing. Through out my nursing career I have been fortunate to work as a RN in Med Surge, however for the past 20 years working as a wound care Nurse. There is nothing like having the opportunity to empower new nurses to overcome their individual barriers to the health field. I have been fortunate to work as Assistant Director of Nursing for the past five years, working in a few rehabilitation facilities where I have met and abundance of talent in the field.
In my spare time, I enjoy being with my family, my dog who I consider as a son, his name is “Charlie”. Not to mention my second love in life is when I am working on my Datsun 1971 vehicle. But above all, my job depicts my true passion to improve the lives of each and every patient I encountered in my path.

Deslandes Cange, RN BSN IP ADON
Asst. Director of Nursing
Phil Schrade ADON, RN, BSN- I have been working in SNF’s for over 10 years. I have held positions such as Nursing Liaison, Assistant Director of Nursing, MDS Assessor, and now Associate Director of Nursing/Facility RN Educator currently at Daleview Care Center.
What I love most about my role working here: The facility is extremely clean, quiet for the most part which is conducive for the resident’s own environment, healing and wellbeing. The food here is excellent. I enjoy interacting with the residents whether they need a cup of water, education on their medications or condition, or just need someone to talk to (I like that I am able to give comfort and set their minds at ease, put a smile on their faces). The staff here is professional, caring, friendly and accommodating. They truly work well together as a team and help each other accomplish care. Whether its camaraderie or helping family members its very gratifying. Many family members feel vulnerable and its nice to know that educating them comforting them, and answering their questions may give them some peace brings me a warm feeling of achievement.

Joanne Mendez RN
Director of Clinical Reimbursement/MDS and Case Management
Hi my name is Joane Mendez Director of Clinical Reimbursement/MDS and Case Management at Daleview Care Center. With over 13years of experience in Medicare and Medicaid Reimbursement, Quality Measures and Utilization Reviews. I am a Registered Nurse with Advanced Certifications in Resident Assessment Coordinator, PRI Certified, International Classification of Disease 10 Coding certified. My goal is to achieve a five star rating in Quality measures while following Medicare and Medicaid guidelines.

Lisa Kemp
Controller
Lisa Kemp is the Controller at Daleview Care Center, where she leads the facility’s financial operations, including accounting, payroll, human resources support, and patient accounts. With over 20 years of experience in Nursing home & long-term care, hospital networks, and non-profit accounting, Lisa is known for her detail-oriented and solutions-driven approach. She holds a Bachelor of Science in Business Administration, a Certificate in Accounting, and an MBA in Organizational Leadership from Eastern University.

Liz Szaluta
Recreation Director
Liz studied English literature at Syracuse University and public health at University of Michigan. She received her Certificate of Dementia Practitioners in 2023. She worked at Bristal and Sunrise previously.
Liz has been in Recreation her whole life. As a child Liz played Rummikub and cards with her grandparents. She enjoyed Scrabble and art projects with her mother, and karaoke with her father and sister. When her father (who was an opera and documentary lover) needed to go into assisted living, and she saw that more diverse programs were needed, she realized publishing (the field she was working in a the time) wasn’t as interesting or rewarding as Recreation, so she switched gears.
Today, Liz is thrilled to be a part of the Daleview Team and looking forward to a fun spring and summer with our Residents, Families and Staff!

Karolina Pieraszek
Director of Food Services & Housekeeping
My name is Karolina Pieraszek, I am certified Dietary Manager with Bachelor’s and Master’s degree in Hospitality, Tourism and Recreation. My hospitality background is an added strength in my current role at a nursing home, where service excellence, empathy, and resident experience are essential. I bring over 20 years of experience in the healthcare with a focus on food service management. Throughout my career, I have served as a Food Service Director in long – term care, assistant living, specialty nursing home settings like kosher facilities, supported dietary operations in dialysis with renal diets, nutritional restrictions, and interdisciplinary coordination is essential.
At Daleview Care Center, I am actively seeking new culinary challenges and opportunities for professional growth. I am focused on introducing new recipes into the daily menu and creating monthly cultural food diversity experiences that allow residents to enjoy a variety of cuisines while respecting individual dietary needs and preferences.
In addition to dietary services, I am in charge of housekeeping operations. monitoring infection control practices, environmental cleanliness standards, and collaborative efforts to support overall facility quality and regulatory compliance. I am committed to maintaining high standards of care across both departments.

Pat Crescione
MSRD, CDN
My name is Pat Crescione MSRD, CDN. I have been employed by Daleview Care Center since 2023 but have been a dietitian since 1999. I grew up in North Bellmore graduating from Mepham high in 1981, then on to college. I am a long island girl at heart and still live on the island. I received my Bachelors in science and nutrition from Queens College, my Masters at NYIT in Westbury and Personal Training Certificate from Hofstra. Registered with Academy of Nutrition & Dietetics and NYS Education Department of Certified Dietitian/Nutritionist. Although I have experience in consulting Gyms, Group Homes, Home Care, Wellness centers and a Private Practice my passion is long term care.

Michael Denegres
Director of Maintenance
I came to work at Daleview Care Center in 1996. Although a lot has changed in these last 30 years I still love working here. I started out as a Maintenance assistant and learned everything I could about the facility how it runs and safety measures. When I took on the role of Daleview Maintenance Director it was like taking a Charger and dropping a Hemi in it. I really felt well connected with the facility and its needs from an environmental and safety perspective.
I also think for our long-term residents when something breaks or doesn’t work, they have more confidence when they see the same face over a period of time fixing and caring for their equipment. I take a lot of pride in knowing that there isn’t one square inch of this facility that I don’t know about, how it runs, how it works or what it needs. I wouldn’t have it any other way.

Rachelle Pierre
Social Work Director
I am a social worker with 17 years of experience serving families and vulnerable populations in New York City. I began my career in child welfare and later worked as a preventive caseworker and housing specialist. I eventually transitioned to working with the geriatric population, where I discovered a true passion for supporting and advocating for older adults.

Jeanette Tannacore
Social Worker
I have been a social worker at Daleview for 2 years and have been working in Health Care for over 25 years. Working with the Geriatric population is not only my career path, but my passion. I work closely with my Residents and their family members to foster a positive and supportive environment. I am committed to advocating for the best interests of my Residents. I take interest in making sure they have the best experience here at Daleview!

Stefanie Mazzocchi
Director of Admissions
I am the Director of Admissions at Daleview Care Center, a role I have proudly held for the past four years. With more then 20 years of experience in skilled nursing facilities, I have dedicated my career to supporting residents and guiding families through what can often be a stressful admissions process with care and compassion, helping them feel at ease during an important transition. I truly enjoy working with people and take pride in helping each resident feel welcomed, comfortable and at home.

This is what I live by, not just as a therapist but as an individual. It is not enough to know what to do or say what to say but to actually roll up your sleeves and get to work. I have been a PT for 20+ years and the joy of seeing a patient take their first steps outweighs the challenges that we face as healthcare providers. Being able to make a difference in their lives in priceless.
